CSBG/LIHEAP Case Manager

THE MISSION
The CSBG/LIHEAP Case Manager is expected to:
· To plan, implement and control GLEAMNS activities involving grants and program operations serving the welfare of residents in fourteen counties.
A SNAPSHOT OF EXCITING THINGS YOU WILL DO:
Allocation of Time for all Items 1-15: CSBG 5%; LIHEAP 95%
· Answer customer’s questions concerning program services.
· Set your own schedule of appointments with clients. Learn all program information as it relates to appropriate bill calculations and determining what programs and/or services are most beneficial to the customer.
· Interview and assess all customers’ information to determine income and program eligibility for LIHEAP programs based on criteria set forth by the funding source. The Case Manager may be held personally responsible for failure to follow criteria set forth by the funding source, as determined by the Program Director and Chief Executive Officer.
· Solicit all proper documentation from customers as outlined by the funding sources to be included in the customer’s files (application, client profile, award, etc.) as well as inputting the information into the DBA FACSPro System.
· Maintain daily logs to ensure that program funds are in balance with the DBA FACSPro System.
· Make referrals to other social services providers; help the customer make contact with appropriate services, follow-up, etc.
· Conduct home visits for homebound and/or wheelchair bound customers if needed, to ensure goals and objectives of the program are being met.
· Represent the agency within the community, attend meetings, speak at functions, etc., to communicate the CSBG’s purpose and progress, and to be an advocate for clients and programs.
· Perform administrative duties and keep accurate documentation of all services provided to clients. Prepare reports as requested.
· Act as an advocate between the client and the vendors. Follow up with the vendors to ensure that the process is completed.
· Participate in the Agency’s internal monitoring of files and systems.
· Make follow-up calls to clients when needed.
· Participate in training(s) provided by the agency, OEO, South Carolina State Association, and/or other entities for personal and professional development.
· Attend scheduled staff and/or zoom meetings.
· Perform other duties as required.

WHAT YOU WILL BRING TO THE TABLE:
· BS/BA Degree in Sociology, Psychology, Social Work, or related field, or a combination of education and experience commensurate with the job description.
· Minimum 3-5 years’ experience in a human service environment working with low-income individuals.
· Knowledge of Microsoft Office software programs.

· Office environment
· Travel, in and out of state, may be required.
· Training and continuing professional development, as required.
SCOPE OF WORK AND ACCOUNABILITY
For the overall development, plan and direction of GLEAMNS activities, including CSBG/LIHEAP Programs.
